Reservation Form Drop-Off

By submitting your Reservation Application here, you are indicating that you have read and understood our policies and agree to comply with these policies as described in the contract and on www.democracycenter.org. You are also agreeing to abide by local, state, and other applicable laws concering drug and alcohol use. We will cancel any scheduled events that advertise breaking our policies or the law.
**Note: We need to receive the completed Contract with the completed Reservation Form to make your reservation!!
Before you upload your form:
- Make sure all the information in the form is correct
- Note that the event title and descprition is what you want others to see on our calendar
- We are growing! Events that are reserved with less than 2 weeks notice are not guaranteed
- Please note that we have a fee for all rooms and all events. All payments must be recieved in order to schedule events.
- Allow 48hrs for us to add your event to our calendar and confirm your reservation.

Please note that we have made changes to our reservation form, if you are using an old version you must download the new form so that your request can be processed. You can find the new reservation form in the "Reserve a room" section. Thank you!
